This is the application form that applicants for Short Term Educational Leave must submit to District Office for approval 3 weeks prior to the event start date. It can be downloaded to your desktop and then printed, signed and mailed to our office via Board courier, ATTN Jennifer Roth. You may also drop off the form in person at 225 Centennial Court, Kitchener during office hours of 8:30am to 4:00pm or placed in a clearly marked envelope, ATTN Jennifer Roth, through the front door mail slot after hours and weekends.
Make sure the form is complete with all required information as listed in the guidelines and required signature(s).
